How to use
The following template complements a well-structured and detailed teaching observation of a peer for the peer-review of teaching process for departmental tenure and promotion process and a Faculty’s teaching dossier. This template is generic and meant to be adapted to suit faculty’s unique needs and disciplinary differences. This template can also be used for formative evaluation, to prepare for summative evaluation. Reviewers are faculty members with strong records in teaching, trained to undertake peer review and are at arms-length from the faculty being reviewed (instructor).

This template is formatted using Microsoft Word so that you can adapt for your departmental needs. It is meant to be used as a title page to accompany your written summary and reflection documents.
